Heel Spurs

Ketoprofen 20% in Transderm-I
-Apply to affected area two to four times daily.

Amitriptyline 2%, Baclofen 5%, Ketoprofen 20% in Transderm-N
-Apply to affected area two to four times daily.

Burning Feet

Amitriptyline 2%, Baclofen 5%, Ketamine 2% in Transderm-N
-Apply to affected area as needed two to four times daily.

Plantar Faciitis

Ketoprofen 10%, Guaifenesin 10%, Lidocaine 2%, Amitriptyline 2% in Transderm-N
-Apply to fascia in the morning and as needed up to four times daily.

Foot Cramps

Guaifenesin 10% in Transderm-N
-Apply to affected area as needed.
-Guaifenesin has muscle relaxant properties when applied topically.

Magnesium 10% in Peppermint Rub
-Apply to feet as needed.

Pain Algorithms

Neuropathy Algorithm

Step 1 (Mild to Moderate Pain)
-Gabapentin 6%, Baclofen 2%, Amitriptyline 2% in Transderm-N

Step 2 (Moderate to Severe Pain)
-Gabapentin 6%, Baclofen 2%, Amitriptyline 2%, Clonidine 0.2%
Lidocaine 5% in Transderm-N

Step 3 (Severe+++ Pain)
-Gabapentin 6% Baclofen 2%, Amitriptyline 2%, Clonidine 0.2%
Lidocaine 5%, Ketamine 5% in Transderm-N

-Oral Dextromethorphan SR Capsules 30mg bid - qid

Recommended Neuropathic Nutritional Support:
-Our best results have been with patients that have combined the above therapies with the following nutritional supplements:
1) Cholecalciferol 25,000 - 50,000 units daily (Orally or Transdermally)
2) Methylcobalamin Nasal Spray or Sulingual 500mcg tid
3) Moxxor® Omega 3 Fatty Acids - 2 caplets daily
4) Vitamin B Complex Sublingual Suspension

Inflammation Algorithm

Step 1 (Mild Inflammatory Pain)
-Ketoprofen 10% in Peppermint Forest Rub

Step 2 (Moderate Inflammatory Pain)
-Ketoprofen 20% + MSM 5% in Transderm-I

Step 3 (Severe Inflammatory Pain)
-Ketoprofen 20%, Piroxicam 1% + Bromelain in Transderm-I

Inflammatory Pain + Muscle Spasm
-Any of the steps above with the addition of Cyclobenzaprine 2%

Recommended Inflammatory Nutritional Support:
-For moderate to severe inflammatory pain we recommend the addition of the following nutritional supplements:
1) Moxxor® Omega 3 Fatty Acids - 2 caplets daily
2)UltraInflamX® by Metagenics® - 2 scoops daily
3) Methylcobalamin Nasal Spray or Sublingual 500mcg bid
4) Compounded Glucosamine/Bromelain/Tumeric Capsules - 1 tid

Opioids/Narcotics

We can help your patients that use narcotics by:

1) Taking any standard oral narcotic drug and formulating it into a sustained release capsule.
-Most drugs in this category are only available in sustained release forms at higher doses. You can now prescribe standard doses in SR capsules.
-Ex. Hydrocodone 5mg SR Caps

2) Reducing the amount of acetaminophen or ibuprofen in combination narcotic therapies for those patients suffering from liver or kidney issues.
-Patients using high doses of combination narcotic/analgesic/anti-inflammatory therapy can risk overdoses of the analgesic or anti-inflammatory component. We can reduce the amount of the analgesic/anti-inflammatory without changing the drug’s schedule category.
-Ex. Hydrocodone 7.5mg/APAP 25mg Caps

3) Changing the dosage form or strength of the drug
-For increased compliance or absorption drugs can be compounded into dosage forms such as: Transdermal gels, Lollipops, Lozenges, Rapid Dissolve tablets, Effervescent powders, etc . . .

4) Adding Dextromethorphan (DM) to affect the patient’s overall tolerance to the narcotic.
-Ex. Morphine 10mg/DM 30mg Caps
