Circulation/Diabetic Toes

Nifedipine 4%/Pentoxyfylline 10% in Arginine Cream

-This combination of drugs includes the vasodilatory effects of the calcium channel blocker nifedipine and the amino acid arginine with the blood flow improvement effects of pentoxyfylline. It has been one of the most popular combinations we have made for patients with circulation issues due to diabetes. We recommend that it is applied to the affected area two to three times daily for optimal results.

Amputation Related Pain

Ketoprofen 10%, Ketamine 5%, Clonidine 0.2%, Amitriptyline 2%, Lidocaine 5%, Guaifenesin 2% In Transderm-N Base

-This formulation should be applied four to five times daily during the first week and then two to three times daily thereafter. If patients are experiencing allodynia at the affected site the addition of a Lidocaine 5% spray may be helpful in addition to the compounded cream.

Wound Care

Ketoprofen 2%, Lidocaine 2%, Phenytoin 2%, and Misoprostol 0.0024% Cream

-Phenytoin has been shown in several studies to increase tissue growth and misoprostol has been used to aid in wound care as a prostaglandin analogue. In combination with pain relievers, this cream applied once to twice daily during dressing changes, has been extremely popular with our wound care specialists.

Diabetic Neuropathy

Gabapentin 6%, Baclofen 2%, Amitriptyline 2%, Clonidine 0.2%, and Lidocaine 5% In Transderm-N Base

-This formulation should be applied four to five times daily during the first week and then two to four times daily for mild to severe pain. Please contact our office for recommendations regarding mild or extremely severe pain options.
